Buffalo Prenatal-Perinatal Network (BPPN) is seeking nominations for our annual Great Baby Beginnings event to be held on July 26, 2017. Below and attached is more information. In advance we appreciate your nomination and sharing this information with others in the community.

Buffalo Prenatal-Perinatal Network’s (BPPN) core mission is to improve maternal and infant health outcomes for high need women and their families, and to help reduce racial and ethnic disparities in those outcomes.

Categories:
Caregiver: A nurse, nurse midwife, nurse practitioner, doula, or doctor.

Organization:
A not-for-profit agency, faith based agency, or clinic practice.

Criteria:
Outstanding contributions to the field of prenatal and perinatal care that is in line with the BPPN mission.

Procedure:
The nominating organization/individual must submit a statement of no more than 500 words detailing why their nominee should be selected.� Any and all details relating to the criteria as outlined above should be included.� Submit a current resume of nominee.

Recognition:
The award recipient will be honored at the Buffalo Prenatal-Perinatal Network, Inc. Board of Directors’ Sponsored Great Baby Beginnings event on July 26, 2017 with an award.
