Buffalo Prenatal Perinatal Network Programs

Healthy Families New York

The Buffalo Home Visiting Program provides comprehensive home based services to families for 3 to 5 years after a child is born. The program has a single goal: to help parents meet the challenges of parenting in order to assure healthy development for their children.

Maternal Infant Community Health Collaborative

This program grant was awarded to Buffalo Prenatal in October of 2013.  The goal of the program is to improve maternal and infant health outcomes for high-need women and to reduce racial, ethnic and economic disparities in those outcomes.  The MICHC Initiative integrates and replaces Buffalo Prenatal’s,  Network and Community Health Workers programs.  However, components of the two programs are used in the new Initiative to help with the development of a community-wide systems based approach to improve maternal and infant health outcomes.

Responsible Fatherhood Initiative

This Initiative helps fathers and families thrive through parenting education, advocacy, mentoring, and job readiness coordination in order to promote responsible fatherhood and enhance the quality of father-child relationships; promoting resiliency and strengthening the family and community infrastructure.
